InCare is seeking a per diem Nurse Practitioner or Physician Assistant to provide hospital medicine services in an inpatient rehabilitation setting. This is an excellent opportunity for an experienced APP looking for flexible scheduling, a supportive practice environment, and meaningful patient interactions.

Responsibilities
- Evaluate and manage patients in the inpatient rehabilitation setting
- Collaborate with physicians, nursing staff, therapists, and case management teams
- Document encounters accurately in the electronic medical record
- Participate in interdisciplinary patient care planning
Qualifications
- Current Nurse Practitioner (NP) or Physician Assistant (PA) license
- Strong clinical assessment and communication skills
- Previous inpatient, hospital medicine, critical care, or rehabilitation experience preferred
- Ability to work independently while collaborating with a multidisciplinary team
