Infection Preventionist & Wound Care Nurse- Dual Role

Key Responsibilities of the Infection Preventionist:
· Develops, implements, and maintains the facility’s infection prevention and control program.
· Ensures compliance with CDC, OSHA, CMS, and state regulations.
· Conducts regular infection control surveillance, audits, and risk assessments
· Monitors infection rates, investigates outbreaks, and recommends corrective actions
· Monitors and keeps antibiotic line listing up to date for every resident
· Monitors and ensures PCC tracking is up to date for the infection control tab
· Monitors and ensures PCC antibiotic time outs are in place and timely
· Provides infection prevention training to staff, residents, and visitors
· Ensures staff understand proper infection control practices including hand hygiene, PPE use and isolation procedures through daily interactions as well as competencies
· Develops, updates, and enforces infection prevention policies and procedures
· Assists and participates in activities related to facility inspections or investigations by regulatory accrediting agencies
· Collaborates with healthcare providers and leadership to ensure best practices in infection prevention are followed
· Monitors new admissions and alerts staff to any infection precautions
· Serves as a resource and mentor for staff, promoting continuous learning and improvement
· Updates patient care plans if necessary in relation to infection prevention
· Ensures PPE and precaution carts are readily stocked and available along with performing weekly rounds to ensure the proper precaution carts are where they belong
· Participates in on call rotation
· All other duties as fit, including filling in for staff development as needed
Key Responsibilities of the Wound Care Nurse:
· Educates patients, family members, and healthcare staff on the products and interventions required for skin healing and maintenance
· Assesses and evaluates patient wounds to determine the appropriate course of treatment
· Collaborates with multidisciplinary team, including physicians to optimize patient care
· Rounds with outside wound nurse weekly
· Updates facility documentation as wounds change and keeps track of all wounds in facility
· Documents and photographs all wounds in facility and ensures they are uploaded within 24 hours of patient’s arrival to patient’s portal and then weekly as necessary
· Updates patient care plan/treatment plan and orders as needed as related to wounds
· Adheres to all healthcare regulations and safety standards
· Stays updated with the latest wound care research, techniques and products
· Cleans and dresses wounds, ensuring sterile technique is followed as necessary and educates staff on new dressing orders as they arise

· An individual in this position may be exposed to bloodborne pathogens, inside and outside environmental conditions, physical, chemical, or respiratory hazards
· This position requires regular movements such as grasping, kneeling, pulling, pushing, reaching, repetitive movement, standing and stooping and an ability to lift up to 35 lbs
· Prior experience working in wound care and infection control preferred
· Wound nurse certification
· Infection Prevention certification
· BLS Certification
· RN License
