Overview

Pine Haven Boys Center is a private, non-profit, non-sectarian residential and educational facility serving boys ages 6–14 who have experienced trauma, neglect, and behavioral or emotional challenges. Located on over 100 acres in Allenstown, NH, Pine Haven provides intensive therapeutic and academic programming to support student growth and long-term success.

Pine Haven Boys Center is seeking a dedicated, highly organized, and family-centered Family Worker to provide comprehensive support, coordination, and advocacy for children and their families. This role serves as a primary liaison between families, internal departments, and external stakeholders and plays a central role in treatment planning, documentation, discharge planning, crisis response, and interdisciplinary collaboration, while supporting families as equal, respected partners in the treatment process.

Provides consistent follow-through with families, staff, and external partners

Key Responsibilities

Caseload Management & Continuity of Care

Manage a caseload of at least eight (8) on-site clients
Maintain responsibility for terminated/discharged cases for six (6) months, including weekly family communication and participation in meetings
Cover cases for other Family Workers during absences as needed

Family Engagement, Advocacy & Communication

Conduct family interviews and home visits at least twice monthly, or more frequently as required
Provide weekly family support through phone calls, emails, and text communication using a company-issued phone
Follow up with families each Monday regarding weekend home visits and with respite families following respite visits
Explain Pine Haven’s program structure and treatment expectations to families; manage concerns, conflicts, complaints, and relationship repair as needed
Notify parents/guardians of incidents; send incident report summaries and respond to follow-up communication
Review Incident Reports with parents and parties to the case and ensure appropriate distribution

Treatment Planning, Documentation & Reporting

Develop treatment plan goals collaboratively with the treatment team
Write and maintain the Family Worker section of all treatment plans
Prepare treatment reports for each child, including the initial 30-day report and subsequent 90-day reports
Implement treatment plans and provide ongoing monitoring and updates
Prepare and organize all intake materials for each child’s file
Develop discharge plans, and prepare all discharge materials and complete full case breakdowns upon discharge
Complete assessments for all family visits and written summaries following meetings
Maintain accurate, timely, and compliant client records, including required filing
Complete Child Information Sheets every six (6) months and distribute after required signatures are obtained

Meetings, Coordination & Interdisciplinary Collaboration

Facilitate and participate in treatment team meetings
Attend placement reviews, court hearings, and IEP meetings, serving as the clinical representative
Write court reports and distribute them at least five (5) days prior to scheduled court dates
Maintain regular communication with internal and external team members, including caseworkers, schools, service providers, and Pine Haven staff

Transportation & Appointments

Transport children to court hearings, medical and mental health appointments, evaluations, and home visits, (sometimes including weekend and extended out-of-state travel (e.g., Vermont))
Coordinate transportation logistics with Safe Passage, etc.

Daily Program Support & Crisis Intervention

Provide therapeutic supervision during family and sibling visits when required
Assist with daily behavior management and program routines
Assist with risk behavior as necessary
Write behavioral and incident reports as required
Maintain visit calendars for assigned clients

Training & Supervision

Attend weekly individual supervision and required staff, clinical, and residential meetings
Participate in required trainings, workshops, and seminars

Additional Responsibilities

Prepare and distribute case-related documentation to all required parties
Assist in managing concerns that arise with parents, guardians, and external stakeholders
Perform other reasonable duties as assigned by the supervisor, particularly during emergencies, to support the safety and welfare of students, families, and the organization

Qualifications

Bachelor’s Degree in Social Work preferred.
Bachelor’s Degree in a related human services field with emphasis in family systems and a minimum of two (2) years of relevant experience may be substituted.
Valid New Hampshire driver’s license required.
Ability to drive Pine Haven vehicles.
Demonstrated verbal de-escalation skills and physical ability to manage risk behavior safely and therapeutically.
Ability to obtain and maintain certification in Crisis Prevention Intervention (CPI), First Aid, and CPR, and other required trainings.
Strong written documentation and organizational skills.

Work Environment

This position is based on-site at Pine Haven’s residential campus and requires extensive interaction with children, families, staff, and external stakeholders. The role involves frequent movement throughout campus, community-based work, transportation responsibilities, crisis intervention, and detailed documentation. Flexibility, resilience, and strong communication skills are essential.
