Overview
We are seeking two dedicated and compassionate Home Care Providers in Epping, NH or the Seacoast region of New Hampshire, or Home Care Providers willing to relocate to this area, to support two adults with intellectual and developmental disabilities in a shared home setting.
This is a full-time, live-in opportunity where both individuals will reside in the providers’ home. The home must include two private, furnished bedrooms—one for each individual. Each bedroom must pass a Life Safety inspection conducted by the local Fire Department.
As Home Care Providers, you will work together to create a nurturing, family-style environment that promotes independence, skill development, and personal growth. Providers will deliver person-centered, 24-hour support tailored to each individual’s needs, strengths, and goals, ensuring a safe, respectful, and supportive living environment.
Job Duties and Responsibilities:
- Provide support to individuals in their daily routines, including tasks such as showering, dressing, grooming, toileting, feeding, medication administration, and range of motion exercises, whether at home or in the community.
- Assist individuals in managing their households and activities, ensuring alignment with the goals outlined in the annual service plan.
- Assist with scheduling appointments and maintaining a personal event calendar.
- Schedule transportation for outings (medical appointments, visiting friends and family, shopping, and attending other religious, community, and social events).
- Help individuals prepare daily meals and snacks, including special dietary meal plans and nutrition assistance.
- Aid individuals in administering their prescribed medications.
- Respond promptly to individuals’ requests for assistance.
- Assist in imparting appropriate social and independent living skills to individuals.
- Foster independence among program participants, guiding them to perform tasks as autonomously as possible and encouraging their maximum participation in all activities.
- Precisely document the care provided as per instructions.
- Stay updated on changes in care and support, including modifications in medications, treatments, and behavior plans.
- Immediately notify program management in the event of any changes in an individual’s condition or behavior.
- Assist in weighing individuals and collect and record other requested data.
- Ensure the timely completion of all documentation and incident reports within the program’s specified timelines.
- Actively engage in meetings involving individuals, families, teams, and staff.
- Attend training sessions as assigned and maintain certification in required areas, including CPR, First Aid, and medication administration, with successful completion of a medication administration competency exam.
Home Care Provider Requirements
To qualify as a Home Care Provider, you must meet the following requirements:
Home Environment
- A private, furnished bedroom available in your home.
- Must meet Life Safety standards and pass a Fire Department inspection.
Screenings & Inspections
- Willingness to undergo background checks(Criminal, DCYF, BAAS, DMV).
- Home inspection compliance.
Transportation
- Reliable transportation for access to appointments, community involvement, and daily needs.
Training Requirements
Completion (or willingness to complete) the following:
- Relias Training
- Medication Administration Training
- MOAB Training (Management of Aggressive Behavior)
- CPR/First Aid Certification
- Any additional state-required trainings for Home Care Providers
Preferred Skills & Experience
- Experience working with individuals with intellectual and developmental disabilities
- Familiarity with positive behavior support, person-centered planning, and trauma-informed care
- Strong skills in communication, patience, and empathy
- Ability to support individuals in making informed decisions and becoming more independent
- Organized, dependable, and able to maintain professional boundaries while providing compassionate care
