Participates as a member of the interdisciplinary healthcare team to facilitate the continuum of care and education of patients with chronic diseases.Utilizes Medical Nutrition Therapy ( MNT)and evidence-based care to provide effective and ethical nutrition counseling topatients.Employsmotivational interviewing techniques to facilitate behavior change.Assess data reflective of patient's nutritional requirements relative to his or her age specific needs required.Collaborates with Population Health for nutrition articles, media programs and /or outpatient programs.
Bachelors degree or higher and completion of an ACEND accredited Dietetic Internship. 3-5 years of experience in nutrition counseling including motivational interviewing. Commission on Dietetics Registration. NH licensure required upon hire.
