We are seeking a compassionate and skilled LPN to join our day shift team at St Johnsbury Center for Living and Rehabilitation, a premier residential, long-term, rehabilitation, and subacute care provider in Saint Johnsbury, Vermont. As a valued member of our team, you will play a critical role in delivering exceptional care to our patients and residents.
Responsibilities:
Provide direct patient care under the supervision of our nursing staff, including administering medications, wound care, and other treatments as needed
Develop and implement individualized care plans to meet the unique needs of each patient
Collaborate with our interdisciplinary team to ensure comprehensive care and promote positive outcomes
Maintain accurate and detailed records of patient care and progress

Requirements:
Currently licensed as an LPN in the state of Vermont
