We are looking for a full-time registered nurse to join our clinical team!
If you are a registered nurse who loves community-based care and education, this may be the job for you! This position includes a mix of in-person and phone triage and support for patients with complex needs. A successful candidate will have a warm and friendly demeanor with patients, be highly organized, confident using EMR systems, and able to think creatively and work well with others. We are looking for a candidate who values building strong relationships with their team of coworkers and patients over the long term.
Responsibilities include:
- Shared responsibility with other members of the nursing team for phone triage and nurse-led face-to-face triage visits during business hours
- Help patients with complex needs access specialist and community resources
- Coordinate care for patients after hospital discharge, in collaboration with the nursing team
- Facilitate our Chronic Care Management Program
- Participate in quality improvement projects
The ideal candidate will have a background in inpatient, primary care, long-term care, or home health nursing. This is a full-time position of 40 hours a week during weekday business hours, with a regular schedule to be established based on candidate and clinic needs.
Required credentials:
- Active NH or Compact RN license
- ADN, BSN preferred
- Current BLS certification
